Emergency Notification for Businesses

The Emergency Notification for Businesses Form helps support communication to businesses and property owners when an emergency occurs to your property after normal business hours and all other times when your business is closed.

In the event of a false alarm, suspicious activity, or otherwise, having a person we can contact helps us to serve you better.

If you are the owner, manager or supervisor of a business within the Champaign city limits, please complete and submit the emergency notification form below.  If you have multiple locations with the same contact person, please list each property address. This information will be used for law enforcement and fire department purposes only.

To maintain accuracy of our records, this form should be completed annually, or whenever there is a change of emergency contact.

    Please provide three names and phone numbers for people in the order that they are to be contacted. These people MUST have keys to the business and preferably live in town. Provide information for three individuals living in three separate households.
