Individuals or groups wishing to propose honorary street name designation are asked to complete the form below, or to complete and return a paper version of the Honorary Street Name Application Form.  All submissions will be received by and reviewed by the Public Works Department.  Individual letters of support are encouraged and should be attached to the application (either electronically or with a paper application).
