Stakeholder Reporting Form

The Champaign Police Department welcomes direct feedback from residents and stakeholders in Champaign, and have created a form intended to provide an open line of communication related to order maintenance and quality-of-life issues. Information submitted through this form is immediately directed to CPD leadership, including District Commanders and on-shift Sergeants. If your concern regards an issue that could lead directly to injury or death, please do not use this form and call 9-1-1 instead. Information submitted through this form may be subject to release under the Freedom of Information Act (FOIA) with or without redaction.
